Risk Assessment

It is a requirement that all people who hire any of the QCA Galleries spaces fill out a Risk Assessment form specific to the activities and circumstances surrounding the installation and display of artwork for exhibition.

The purpose of this form is to identify any risks or concerns associated with your install and/or artworks so that appropriate measures may be taken to minimise any harm or injuries.

All questions marked with a red asterisk* require a response.
