Being at risk of a chronic condition can be due to a number of different factors, not just weight. Your score is made up of things you can't change, such as your age, gender, ethnicity, family history and genetics, and some things you can. These are often referred to as lifestyle factors, or choices. These include what and how much we eat, the amount of physical activity we do each day, risky alcohol consumption, and smoking. By modifying or making positive lifestyle choices may help reduce your overall risk and improve your long-term health.

After completing the six program sessions, you will be automatically eligible for our Maintenance program. This is a digital offering with online tools and resources for you to access whenever you need. There is also the option of enrolling in the program again (plenty of participants have!).
